Step 1: Fill your Dog History Form out and hit the continue button.
Step 2: Create your account, sign contracts, and add your dog(s) to our system
Step 3: We will need your dog’s most recent vaccines based on the services you will be utilizing. You can either upload your vet copy online via your owner portal or email them to us to info@mydoginc.us
Grooming: Rabies & Distemper
All Other Services: Rabies, Distemper, Bordetella (annual), and Fecal Report (annual, with negative results)
Step 4: Once the above steps have been completed, please contact us to confirm that we have received all the necessary documents to schedule services.
